To be eligible, all you need to do is make at least one deposit during the month and then fill in the form below. There is a limit of one entry per day. The winner can choose to accept the awarded monthly prize or $100 in cash. The monthly prize cannot be exchanged. Contest duration: between October 1st, 2008 and September 30th, 2009.
